Yes, this is unusual, I've not seen one printed like this before. But I think it's supposed to be like this, maybe that's what a 'cheat sheet' means.... a kind of a mix between lyric/chord and some notation.
Not so useful for me but it could be for some, those who don't need all the notes to get through a piece!
